Shero Agreement
The undersigned individual hereby indicates their desire to become a member of Shero Taekwondo, pursuant to the terms and conditions of this membership agreement.
Health Attestation
The Member attests that they are fully able to participate in an exercise regimen of Taekwondo training and choosing without undue risk.
Membership Dues and Payment Schedule
Payment is made monthly (not one-time/advance). The Member's official renewal date is based on the date of their first class. Monthly sessions must be completed within the same billing cycle (within 30 days from the first class date). Unused sessions do not roll over and are not refundable, unless there is a valid reason (e.g., medical or emergency) and an extension is approved by SHERO.
To help keep accounts updated, the Member will receive an automatic invoice via email on the next renewal date (the following month). Payments should be settled on or before the renewal date each month. Payments may be made via online banking, GCash, or cash in person.
Member Access
This membership agreement entitles the member to access Shero Taekwondo classes with the respective class hours so long as all dues are current
Membership Cancellation
This membership may be cancelled by providing written notice, or in person, at Shero Taekwondo. If the membership contract is cancelled prior to agreement expiration date, the Member agrees to pay a cancellation fee of 20% of the remainder dues. If the Member must cancel this membership agreement due to relocation to an area more than 10 kilometers from a Shero Taekwondo studio, the Member may cancel without paying the cancellation fee.
Media Release & Consent
As part of SHERO's mission to empower students and build community, we regularly capture photos and videos during classes, events, and special activities. These materials may be used for promotional, educational, and documentation purposes.
Media Opt-Out: If I prefer my child to not be included in any media materials, I will notify SHERO in writing via email: info@shero.com.ph
WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND RELEASE AGREEMENT
I, the undersigned, understand and acknowledge that participation in Taekwondo or any physical activity involves inherent risks of injury. These risks include, but are not limited to, slips, falls, collisions, contact with other participants, and physical exertion.
By signing this form, I agree to the following:
- Voluntary Participation
a. I understand the nature of the activity and choose to participate (or allow my child to participate) voluntarily.
- Assumption of Risk
a. I fully assume all risks, known and unknown, related to participation in the SHERO Taekwondo trial or drop-in class.
- Release of Liability
a. I hereby release and discharge SHERO Taekwondo, its coaches, owners, agents, and staff from any and all claims, liabilities, or causes of action resulting from injury, loss, or damage that may arise from participation.
- Medical Authorization
a. In case of an emergency, I authorize SHERO staff to seek necessary medical treatment for my child or myself. I understand I will be contacted immediately.
Liability and Risk Acknowledgement
By signing this agreement, I acknowledge and fully understand that participation in Taekwondo classes and related physical activities at SHERO Taekwondo involves inherent risks. These may include, but are not limited to: physical contact, accidental collisions, slips, falls, muscle strains, sprains, fractures, or more serious injury.
I hereby agree to the following:
- I understand and accept the potential physical risks associated with martial arts training.
- I voluntarily assume full responsibility for any injury, accident, or health-related issue that may occur while participating in any SHERO class, activity, or event.
- I release, waive, and discharge SHERO Taekwondo, its coaches, staff, volunteers, and facility partners from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury sustained.
Acceptance
The below signed parties hereby enter into this membership agreement with one another in acknowledgement and acceptance of the terms listed above.
